12. +3 Pass Certificate (For MCA, MBA).
13. Final seat allotment letter.
14. Admit card (JEE).
15. Payment slip college copy.
16. Verification slip.
17. Passport size photograph. (04 Numbers) (Preferably color).
18. Demand Draft In favor of " S M Charitable Educational Trust " or " National Institute of Science & Technology "
    payable at any nationalized bank at Berhampur, Odisha.
